Every entry, newest first

Nothing is removed after the fact. Losses, breakevens and cancelled plans are logged in the same detail as the entries that reached target — an entry the method got wrong teaches more than one it got right.

Anatomy of an entry

Every entry uses the same eight fields in the same order, so two setups months apart can be compared on equal terms.

Entry
The price area where the plan becomes actionable — a band rather than a single tick, because fills are never surgical.
Stop loss
The price that proves the idea wrong. If it trades there the plan is closed and the loss is taken in full, not widened.
Targets
Exits chosen before entry, so taking profit is a decision made in advance rather than one made mid-candle.
Planned R:R
Distance to target measured against distance to stop, as written at entry. It describes the plan, never the result.
Outcome
What actually happened, in R. One R is the amount risked, so results stay comparable across markets and position sizes.
Invalidation
The condition that voids a setup before entry — separate from the stop, which only applies once a position exists.
Thesis
One statement of what has to be true for the idea to exist, written before entry so it cannot be rewritten afterwards.
Lesson
The review written once the outcome is known — including on the plans that never triggered at all.
